Survey Form

You can create a Survey Form from Survey Form -> Survey Form menu from sidebar. There are three type of modules to create survey from :

1. Forms
2. Review Indicators
3. Survey Form

Forms

Forms are predefined templates containing structured questions designed to collect employee feedback. You can create and manage these forms from the Survey Form → Forms page.

To create a form template, fill in the following required fields:

• Name, provide a clear and concise name for the form (required).
• Status, set the form as Active to make it reusable in surveys or Inactive to disable its use.
• Form Fields, define the survey questions using a repeater to structure the content efficiently.

Review Indicators

Review Indicators are predefined templates containing structured topics designed to collect employee review. You can create and manage these review inidicators from the Survey Form → Review Indicators page.

To create a review indicator template, fill in the following required fields:

• Name, provide a clear and concise name for the review indicators (required).
• Form Fields, define the indicator topics using a repeater to structure the content efficiently.

Survey Forms

A Survey Form is a structured feedback form containing questions designed to gather employee reviews or feedback. You can create and manage survey forms from the Survey Form → Survey Form menu from sidebar.

A survey form is divided into three key sections:

1 Basic Info -To set up a survey form, complete the following required fields:

• Title, provide a clear and concise title for survey form (required).
• Last Date, specify the final submission date for survey responses(required).
• Visible To, choose whether the survey is open to all employees or select specific employees.
• Employees, If selecting specific employees, choose multiple employees to participate (required).
• Description, add any additional details or context about the survey.

2 Feedback Form -In this section, select a predefined form template to insert structured questions for collecting feedback. 3 Review Indicator Form -Here, choose the appropriate review indicators to include relevant evaluation topics in the survey.
